Medical Power of Attorney

A Medical Power of Attorney lets you name a health care agent to make medical decisions on your behalf if a physician certifies that you can no longer make them yourself.

What this document does

This questionnaire is for the Texas statutory Medical Power of Attorney form.
You enter the principal’s name, a primary health care agent, and optionally up to two alternate agents who serve in order.
When you finish, the app generates a completed Texas Medical Power of Attorney Word document for review and signing.
