Medical Power of Attorney

A Medical Power of Attorney lets you name a health care agent to make medical decisions on your behalf if a physician certifies that you can no longer make them yourself.

What this document does

  • This questionnaire is for the Texas statutory Medical Power of Attorney form.
  • You enter the principal’s name, a primary health care agent, and optionally up to two alternate agents who serve in order.
  • When you finish, the app generates a completed Texas Medical Power of Attorney Word document for review and signing.
