Directive to Physicians

A Directive to Physicians (also called a living will) lets you state your treatment preferences if you have a terminal condition or irreversible condition and cannot speak for yourself.

What this document does

  • This questionnaire is for the Texas Directive to Physicians and Family or Surrogates form.
  • You enter the individual's name and county, and optionally designate up to two health care spokespersons if no Medical Power of Attorney has been executed.
  • When you finish, the app generates a completed Texas Directive to Physicians Word document for review and signing.

When to use

Use this when a client wants to express their end-of-life treatment wishes in advance under Texas law.
